What should I bring with me when I come for an appointment?

• A Driver’s License or another official picture ID
• Insurance information
• Referral Letter (if required)
• Reports, X-rays, MRI’s CT scans etc. and any other relevant information
• List of medications (if any)

Do you have Physical Therapy in Office?

Yes we have an in house physical therapy specialist.

Do you have an MRI in the office?

Yes we have an in house MRI.

How do I cancel an appointment?

Please call the office during business hours and give at least one business day notice.
